VON - At the heart of care. *About the role:* The Community Navigator role bridges gaps in access to support for underserved and ethnically diverse communities by identifying community needs and connecting individuals to appropriate services. This work is guided by a commitment to cultural humility and strengthening connections through meaningful interactions with community members, ensuring that all activities, consultations, and service delivery are approached through a culturally responsive lens. *What we offer:* Competitive salaries, vacation time, and paid statutory holidays Tiered Benefits (health, dental, LTD) and an Employee Assistance Program Defined Benefit Pension Plan with company contribution Comprehensive orientation program Opportunities for skill development, eLearning, and career advancement *What you'll do:* Develops and executes strategies to recruit program participants Attends community events and festivals, and delivers presentations with cultural humility to increase awareness of and promote health and wellness programs Collaborates with partners to support communities and expand referral resources Creates culturally appropriate messaging and uses interpretation services for outreach Surveys diverse communities to assess access and barriers to care Identifies systemic needs and co-creates community-driven solutions Collects data for formal program assessments with clients and other health partners to ensure programs meet community needs Uses community feedback to improve outreach, volunteer recruitment, and fundraising efforts *What we're looking for:* Bachelor’s degree in social or health sciences, education, communications, or a related field Minimum 3 years of proven experience in canvassing, outreach, and data collection Minimum 1 year of experience in project planning, coordination, and reporting Demonstrated experience working with ethnically diverse populations Demonstrated experience working with external partners and volunteers Please use this link for applying: https://jobs.jobvite.com/von/job/o1pjzfw2
